We are now taking memberships/renewals for the 2010-2011 school year. Please print this form, fill it out, and mail it with the appropriate membership fee. Once we receive your membership form we will email you a username and password to access our member support forum and calendar of events. If you are renewing your information will stay the same.

**You may also pre-order 2010-2011 yearbooks with your membership fees. If you preorder along with your registration fee the cost is $20 per yearbook. The price will go up to $30 once the Aug. 1st registration deadline ends so take advantage. If you would like to preorder please include $20 per yearbook when mailing your member fees. |

We are in agreement with C.H.E.V.A.'s Statement of Faith.
- We are regular attenders with a local Christian Church.
- We commit to be an active member by participating in field trips, project days, and community service days that the leadership schedules.
- Out of respect for all involved in offering activities, we will immediately and in a timely manner notify the contact person of an an event we are unable to attend that we had previously committed to.
- If a field trip requires a fee, I will send it in a timely matter.
All fees are non refundable.

Waiver of Liability
In the event of an accident or injury, I will assume full responsibility for my child(ren) named below, thereby releasing the leadership of Christian Home Educators of the Visalia Area known as C.H.E.V.A. of any obligation or responsibilities.
